The form is now available for download from the Discussion page. As Sally has yet to email me a version of this year’s Form, I’ve edited last year’s one, but this should be identical to the “official” version.

NB: This “News” post was not supposed to be visible (yet) – it’s required for a new design of the Home page, to be launched later this summer once the screening dates and the order the films are shown has been finalised. Thank you for revealing the mistake in the link to the discussion threads.
